Corrective Action Guide: From Safety Findings to Effective Action

This corrective action guide explains how to turn incidents, inspections, audits and other safety findings into actions that address meaningful causes and produce verifiable improvement.

What Is Corrective Action?

Corrective action addresses a detected problem and relevant causes to reduce the likelihood of recurrence. It should be distinguished from simply correcting the immediate visible condition.

Define the Problem Clearly

Describe what occurred or what condition was identified. Poorly defined problems often lead to weak actions that do not address the underlying issue.

Understand Contributing Factors

Use relevant evidence to examine why the problem occurred. Consider controls, equipment, processes, procedures, supervision, communication and organizational factors where relevant.

Select Strong Corrective Actions

Choose measures that address significant contributing factors. Where risk controls are involved, consider higher-order measures before relying mainly on reminders, warnings or retraining.

Assign Ownership and Priorities

Define who is responsible for implementation and prioritize actions according to risk and urgency. Complex actions may require milestones rather than one final completion date.

Verify Effectiveness

Completion does not necessarily mean the problem has been solved. Review whether the action was implemented as intended and whether it produced the expected improvement.
